I lined the buttons up on the card.  Once I got the placement correct - i took a black marker and made a dot in 2 circls on each button - to hold it's place.  I than removed the buttons (still keeping them in a line, but off the card) and pierced the paper where I drew the black dots.  I than took the floss and threaded it through each button, tieing it off.
